Kids ages 15-18 years are invited to join in on some soccer fun! Learn the game with PHSC Head Soccer Coach, Mark Leonard at Wesley Chapel District Park. The registration fee is 75$ per camper.

Permission to secure treatment

I hereby request that my child/camper be admitted to PHSC Women's Soccer Camp and authorize the directors or PHSC Camp Staff to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for me or my minor child/ward’s immediate care and agree that I will be responsible for payment of any and all medical services rendered. Additionally,  I release from liability and hold harmless from any and all claims for loss of property, personal injury, or death sustained by my camper arising out of any camp activities conducted at PHSC and I will be required to sign a complete waiver of liability before my child can attend the first day of camp.
