Instructions and Important Information
Note: All applicants must apply to the College prior to applying to a PHSC limited access program.
Read all requirements and complete and submit this online limited access application.
Applicants must submit all selected admission program requirements to be considered for review. Applicants who submit only part of an application or are missing required documents will not be processed, must reapply to the program, and repay the selected admission application fee once all requirements are met.
Applicants who are offered a seat in the program and fail to respond by the date indicated on the offer letter or who fail to enroll for the term for which admission is offered must reapply for a future date.
Applicants who meet minimum requirements as of the deadline date will be selected based upon essay review.
I understand that I am responsible for submitting a complete application by the stated deadlines and submitting all required documents and that incomplete applications are not considered.
Requirements
- Submit a PHSC general admission application with a $25 non-refundable fee for new students or a PHSC readmission application for returning PHSC students (no additional fee) not enrolled for more than 1 year.
- Submit a Bachelor of Science in Nursing program online application. The BSN application is only available during the application cycles.
- Submit a payment receipt copy, in PDF format, of the separate $25 non-refundable selected admission BSN application fee payable to Financial Services.
- Submit a clear, enlarged copy, in PDF format, of current driver’s license.
- Submit a clear, enlarged copy, in PDF format, of current Florida Registered Nursing license.
- Submit the BSN Philosophy of Nursing Education and Clinical essay in PDF format. The Philosophy of Nursing Education and Clinical essay requirement and template are available on the BSN program page.
- Submit an official high school transcript with graduation date indicating receipt of a standard high school diploma or an official transcript of GED scores indicating receipt of a high school equivalency diploma, if not already submitted.
- Submit official transcripts from each U.S. college and university attended other than PHSC.
- Associate in Science registered nursing degree from a regionally accredited institution or a registered nursing diploma from a school accredited by the Accreditation Commission for Education in Nursing (ACEN) or the National League of Nursing Accrediting Commissions (NLNAC).
- Submit an official transcript that shows awarded nursing degree/diploma other than PHSC nursing graduates.
- A cumulative grade point average of 2.5 or higher.
- College level math readiness. Student must meet one of the following requirements:
- Complete MAT 1033, MAT 1100 or higher-level college level math course with a grade of “C” or higher.
- Current placement test score that verifies student is eligible to register for any area II college level math course.
- College level reading and writing. Student must meet one of the following requirements:
- Complete ENC 1101 English Composition I or higher-level course with a grade of “C” or higher.
- Current placement test score that verifies student is eligible to register for ENC 1101.
Additional Information
Please note that official high school, GED or college transcripts must be sent to our Admissions Office in New Port Richey in a sealed, unopened envelope or emailed to admissions@phsc.edu from the sending institution. PHSC will not accept emailed transcripts from the student.
Special note about fees:
The BSN application fee is separate from the general application fee. Payment can be made at the West Campus to the Financial Services office in Building E on the first floor.
Additionally, a copy of driver's license, Florida RN license and BSN Philosophy of Nursing Education essay must be submitted in one email in a PDF format to lahapps@phsc.edu, or delivered in person to the Admissions office at the West Campus in New Port Richey within 7 days of submitting this web application, or the program application deadline, whichever is sooner. Documentation must include your name, student ID number and program major.
